This book selectively discusses recent advances in human experimental models relating to the treatment of opioid dependence. The author critically reviewed three independent lines of research conducted during the past decade with heroin-dependent volunteers. One research strategy involves studying the -opioid receptor (OR), which is the molecular target for the reinforcing and physical dependence producing effects of heroin-like drugs. This has been accomplished by varying medication dose conditions, measuring the availability of brain -receptors in vivo and plasma pharmacokinetics, then correlating these biological concentration measures with clinically relevant endpoints, including opioid withdrawal symptoms, heroin craving, and blockade of the euphoric and respiratory depressant effects of -agonist challenges. These studies provide an initial benchmark for estimating OR occupancy or plasma concentration requirements for effective pharmacotherapy. A second research strategy determines the conditions under which -agonist medications function as reinforcers, and the ability of medication dose and the availability of non-drug alternative reinforcement to attenuate opioid choice. Taken together, these studies provide information relevant to the ability of the medication to stimulate adherence and reduce drug demand. A third research strategy seeks to determine whether non-opioid medications can attenuate the naloxone-precipitated (i.e. withdrawal related) discriminative stimulus, subjective and/or physiological effects in opioid-maintained individuals. These studies aim to identify compounds that may be safe and effective adjuncts during opioid detoxification and, potentially, anti-relapse agents. Evidence is also reviewed concerning individual differences identified in these models. Implications of these findings for clinical treatment are also discussed.Author: Mark Greenwald Paperback: 82 pages Company: Nova Science Pub Inc (2008-10) ISBN: 1604568070 List Price: $39.00 Amazon Price: $14.50 Used Price: $23.89 This digital document is an article from Family Practice News, published by International Medical News Group on January 15, 2005. The successor to Strain and Stitzer's Methadone Treatment for Opioid Dependence (Johns Hopkins, 1999), this expanded and updated volume reflects new developments in treatment protocols. Methadone is still the most widely used medication for the treatment of opioid dependence, and the authors provide an extensive section on methadone treatment. Three chapters cover the pharmacology and clinical use of buprenorphine as well as the latest research on Naltrexone, Clonidine, and Lofexidine. The volume also includes chapters on pain and prescription opioids as well as medication-free treatment and medically supervised alternatives to opioid substitute treatments, including withdrawal. The Treatment of Opioid Dependence will be a valuable resource for methadone counselors, psychiatrists, psychologists, social workers, mental health nurses, and addiction counselors, as well as physicians interested in office-based buprenorphine treatment. The most comprehensive text of its kind, this book addresses both the management of opioid withdrawal and opioid maintenance therapy, and combines up-to-date reviews of the scientific literature with evidence-based clinical guidelines for all of the relevant pharmacotherapies. This volume, is developed from a collaborative study sponsored by the World Health Organization over a period of five years, the result of the combined effort of over 30 experts working in 19 different countries. The book presents a comprehensive survey of the development, policy, and procedures of methadone maintenance in countries with experience in this treatment. In addition to presenting a general overview of methadone's role in treating opioid addiction, the volume also focuses on specific questions of current interest, particularly the critical issue of methadone maintenance in the prevention of AIDS. Public health specialists and policymakers will find this book an invaluable source of information about what can be expected from methadone maintenance programs and why such programs have achieved some success in the rehabilitation of opiate users, while so many other attempts in this area have failed. Following an introductory chapter, the contributors look at the impact of AIDS epidemiology on methadone policy. The role of methadone in addressing intravenous drug abuse and unsafe sex practices in drug dependent persons is explored in an attempt to assess the ability of methadone maintenance programs to stem the spread of AIDS. Subsequent chapters review the world literature on methadone in treating opioid dependency, explore national policies and practices regarding methadone maintenance, present an international survey of methadone's role, and analyze the present state of research on methadone. A bibliography and index complete the volume.Hardcover: 128 pages Company: Praeger Publishers (1990-06-26) ISBN: 027593392X List Price: $87.95 Amazon Price: $87.95 Used Price: $63.05
Throughout the world, hundreds of thousands of people are addicted to opiates. The human, economic, and societal costs of this addiction are staggering: more than one-quarter of prison inmates are incarcerated for drug offenses and there has been a dramatic increase in the prevalence of HIV and hepatitis among intravenous drug users. Methadone Treatment for Opioid Dependence examines various aspects of the most widely used medication for the treatment of opioid dependence. Used clinically for more than thirty years, methadone is the most common pharmacological treatment for opioid dependence. Some 115,000 people are actively enrolled in methadone programs in the United States, and estimates range as high as 250,000 worldwide. Methadone eases withdrawal symptoms and blocks the sensation of "high" if the person should take an opioid. But as the authors note, methadone treatment is more than simply the delivery of medication -- it is a combination of pharmacologic and nonpharmacologic treatments. The latter can include individual and group therapies, urine testing, education and vocation training, medical and psychiatric treatment of co-morbid conditions, and other services appropriate to the needs of the patient. When delivered properly, methadone treatment can be highly effective in decreasing illicit drug use and increasing prosocial behaviors in patients. The authors -- all faculty members of the Johns Hopkins Medical Institutions -- provide a historical overview of methadone treatment as well as a review of the regulations associated with the treatment. While important principles and theory are thoroughly examined, the emphasis throughout is on such practical issues as dosing, urine testing, detoxification schedules, and contingency management. Other topics include individual and group counseling, behavioral treatments, and special issues for women. Finally, the book provides comparative information about the utility of other pharmacotherapies for opioid dependence, such as LAAM, buprenorphine, naltrexone, clonidine, and lofexidine. Methadone Treatment for Opioid Dependence is a focused and comprehensive reference for drug treatment policy makers, as well an invaluable resource for professionals who treat opioid substance abusers -- psychiatrists, psychologists, social workers, mental health nurses, and addictions counselors. Paperback: 352 pages Company: The Johns Hopkins University Press (1999-06-10) ISBN: 0801861373 List Price: $28.95 Amazon Price: $132.00 Used Price: $10.99
This state-of-the-art volume integrates chapters on the scientific basis of opiate addiction with a comprehensive survey of the latest treatment methods, including traditional and new pharmacotherapies, adjunct therapies, and management of comorbid substance abuse and medical conditions. An opening section reviews the fundamental neurobiological principles guiding clinical intervention. Chapters then describe the impact of recent scientific breakthroughs on the success of traditional pharmacotherapy, focusing on the evolution of methadone maintenance programs - the primary treatment for opiate dependence for over 30 years. The book also describes and evaluates ancillary services such as psychosocial interventions, day programs, and acupuncture; considers the specific treatment needs of diverse populations; and explores cutting-edge developments in the field. This book will be of interest to psychiatrists, psychologists, and other professionals working with substance-disordered patients, as well as students in these areas.
