The research on mental illness shows that anxiety disorders are quite common. For example, adults with one or more anxiety disorder are the largest group of mentally ill persons in this country. This study examined social anxiety in 81 patients with BDD and change in social anxiety with pharmacotherapy. Subjects completed the Social Avoidance and Distress Scale (SADS) and were assessed with measures of BDD symptomatology. Participants in a placebo-controlled fluoxetine trial completed measures at baseline and endpoint. The mean SADS score was 1.3 SD units higher than nonclinical sample means but consistent with other clinical sample means. Social anxiety was significantly correlated with BDD severity. Greater depressive symptoms as well as comorbid avoidant personality disorder, but not comorbid social phobia, were also associated with higher SADS scores. Social anxiety did not improve more with fluoxetine than placebo, yet it improved significantly more in fluoxetine responders than in nonresponders. He grew up with a mother that hardly ever left her home and when she did, she tried her best to make herself as inconspicuous as possible, and e try not to speak at all unless she definitely had to. He never got accustomed to the fact that albeit she was basically fine in their own house, that she was not able to work outside the house or to develop any type of meaningful relationships with other people, even though they tried to reach out to her in order to initiate a friendship. He was resigned to what he thought was the fact that his mother was just extremely shy and he would have left it at that until he went to a family reunion and heard about a distant cousin who was the same way as his mother. He never got to meet her at the reunion however, as she ted no to turn up at all. He met her many years later, but had to visit her at home, and although he found her to be highly intelligent, she didn’t have a lot to say and she turned down any overtures from him to come to future family reunions or to try to get a job so that she can come out and live her life in the open rather than mostly at home. Two hundred and twenty-eight consecutive outpatients (181 females and 47 males) with a primary diagnosis of binge eating disorder were reliably assessed with diagnostic interviews. Internal consistency of AVPD criteria was good, as suggested by coefficient alpha of 0.87, the pattern of inter-item correlations (range 0.41 to 0.64), and the lack of changes in alpha if any criteria are deleted. Exploratory factor analysis revealed a one-factor solution (56% of variance) supporting the unidimensionality of the AVPD criteria. Diagnostic efficiency indices (conditional probabilities, total predictive power, and kappa coefficients) were calculated for each AVPD criterion, for the entire study group and separately by gender. Overall, the best inclusion criterion was 'fears being ridiculed,' which was also the best predictor overall. These psychometric findings did not differ by gender. The Minnesota Multiphasic Personality Inventory (MMPI, MMPI-2, and MMPI-A) and the Rorschach are the two tests used most widely in clinical personality assessment to evaluate personality functioning, current emotional state, and the presence, nature, and severity of psychopathology, as well as to formulate treatment interventions.
