From the author: Schedule-induced polydipsia in rats was developed by means of a fixed-time 60-s schedule of food presentation. The acute administration of d-amphetamine sulfate (0.1-3.0 mg/kg) produced a dose-dependent decrease in the rate of licking. d-Amphetamine shifted to the left the temporal distribution of adjunctive drinking within interfood intervals. Dopaminergic antagonists SCH23390 ([R(+)-7-chloro-8-hydroxy-3-methyl-1-phenyl-2,3,4,5-tetrahydro-1H-3-benzazepine hydrochloride], 0.001-0.01 mg/kg), eticlopride (0.001-0.03 mg/kg), and flupenthixol (0.03-0.3 mg/kg) also produced dose-dependent decreases in licks per minute but did not alter the temporal distribution of adjunctive behavior. Selective doses of dopaminergic antagonists further suppressed licking rates when combined with d-amphetamine, 1.0 mg/kg, but an antagonism could be observed on the temporal distribution after the administration of SCH23390 (0.01 mg/kg) or flupenthixol (0.1 mg/kg). The dopaminergic agonists SKF38393 ([R(+)-1-phenyl-7,8-dihydroxy-2,3,4,5-tetrahydro-1H-3-bezazepine hydrochloride]; 1.0-10.0 mg/kg) and quinpirole (0.003-0.03 mg/kg) produced a dose-dependent decrease in the rate of licking, and when combined with d-amphetamine, additive-like suppressive effects were found. These results suggest a dopaminergic selectivity for the effects of d-amphetamine on the temporal distribution of schedule-induced polydipsia.

This trial of cognitive-behavioural therapy (CBT) based amphetamine abstinence program (n=507) focused on refusal self-efficacy, improved coping, improved problem solving and planning for relapse prevention. Measures included the Severity of Dependence Scale (SDS), the General Health Questionnaire-28 (GHQ-28) and Amphetamine Refusal Self-Efficacy. Psychiatric case identification (caseness) across the four GHQ-28 sub-scales was compared with Australian normative data. Almost 90% were amphetamine-dependent (SDS 8.15+/-3.17). Pre-treatment, all GHQ-28 sub-scale measures were below reported Australian population values. Caseness was substantially higher than Australian normative values {Somatic Symptoms (52.3%), Anxiety (68%), Social Dysfunction (46.5%) and Depression (33.7%)}. Improvements in Executive Function Correlate with Enhanced Performance and Functioning and Health-Related Quality of Life: (Postgraduate Medicine) Objective:

To assess whether improvement in executive function correlates with perceived improvement in health-related quality of life (HRQOL) in adults with attention-deficit/hyperactivity disorder (ADHD) who are treated with stimulant medication.

Methods:

We collected data on executive function and HRQOL using the Brown Attention-Deficit Disorder Scale (BADDS) and the ADHD Impact Module–Adult (AIM-A) during 2 large, randomized, double-blind, placebo-controlled trials evaluating the use of the triple-bead mixed amphetamine salt (MAS) SPD465 as stimulant pharmacotherapy for ADHD. BADDS and AIM-A data were collected at baseline and at study endpoint, and changes from baseline were assessed. Correlation coefficients for BADDS and AIM-A were calculated at baseline and endpoint for each study.

Results:

The change from baseline in BADDS total score was −14.8 ± 24.49 in Study 1 (dose optimization strategy) and −26.1 ± 28.54 in Study 2 (forced–dose titration strategy), demonstrating substantial reported improvement in executive function. In both studies, improvement was reported for all 5 BADDS clusters at study endpoint versus baseline. While improvement was reported for all 6 AIM-A scales at study endpoint versus baseline, the most substantial change from baseline in AIM-A scores over the course of the studies was in Performance and Function (Study 1, +15.2 ± 25.68; Study 2, +26.3 ± 28.17). There was significant correlation of the BADDS total score with the AIM-A Performance and Function scale (Study 1, −0.6723; Study 2, −0.6982), and strong correlations were observed between each of the 5 individual clusters of BADDS and AIM-A performance and function in both studies.

Conclusions:

Reported improvement in executive function correlates with reported improvement in HRQOL as assessed in 2 independent clinical trials in which participants received either placebo or triple-bead MAS for the treatment of ADHD. In particular, there were strong correlations between all 5 individual clusters of BADDS and the AIM-A Performance and Function scale. To examine duration of efficacy of lisdexamfetamine dimesylate (LDX) in adults with attention-deficit/hyperactivity disorder (ADHD) by effect size in performance and symptom improvement in a simulated adult workplace environment (AWE).

Methods:

Adults (aged 18–55 years) with ADHD enrolled in the AWE study of LDX with open-label dose-optimization and randomized, placebo-controlled, double-blind, 2-way crossover phases. Efficacy measures included the Permanent Product Measure of Performance (PERMP)-Attempted (-A) and PERMP-Correct (-C) scores assessed throughout the day and the ADHD Rating Scale IV (ADHD-RS-IV) with adult prompts. Model-based least-squares (LS) mean effect size was assessed for PERMP and post-hoc ADHD-RS-IV with adult prompts. Remission was defined as an ADHD-RS-IV total scores ≤ 18. Safety assessments included treatment-emergent adverse events (TEAEs) and vital signs.

Results:

Least-squares mean (standard error [SE]) effect sizes were 0.9 (0.17) for PERMP-A and 0.8 (0.16) for PERMP-C for all postdose sessions. For PERMP-A, postdose LS mean (SE) effect sizes were 0.5 (0.15), 0.8 (0.16), 0.7 (0.16), 0.7 (0.16), 0.7 (0.16), and 0.6 (0.16) at 2, 4, 8, 10, 12, and 14 hours, respectively. Medium-to-large effect sizes (0.5–0.8) were generally maintained from 2 to 14 hours for all PERMP assessments. Overall LS mean (SE) ADHD-RS-IV total and subscale effect sizes were −1.2 (0.19), −1.2 (0.19), and −1.0 (0.17), respectively. Remission was achieved in 67.6% of participants receiving LDX. Treatment-emergent adverse events (≥ 5% with LDX) during the 4-week dose-optimization phase were decreased appetite, dry mouth, headache, insomnia, upper respiratory tract infection, irritability, nausea, anxiety, and feeling jittery. During the crossover week on LDX, there were no TEAEs ≥ 5%.

Conclusions:

In adults studied in the AWE, medium-to-large model-based effect sizes were maintained from 2 to 14 hours postdose, on a performance-based measure of productivity, suggesting participants experienced improvement in sustained attention throughout the day and into the evening hours. Lisdexamfetamine dimesylate demonstrated a safety profile consistent with long-acting stimulants.

From the author: Schedule-induced polydipsia in rats was developed by means of a fixed-time 60-s schedule of food presentation. The acute administration of d-amphetamine sulfate (0.1-3.0 mg/kg) produced a dose-dependent decrease in the rate of licking. d-Amphetamine shifted to the left the temporal distribution of adjunctive drinking within interfood intervals. Dopaminergic antagonists SCH23390 ([R(+)-7-chloro-8-hydroxy-3-methyl-1-phenyl-2,3,4,5-tetrahydro-1H-3-benzazepine hydrochloride], 0.001-0.01 mg/kg), eticlopride (0.001-0.03 mg/kg), and flupenthixol (0.03-0.3 mg/kg) also produced dose-dependent decreases in licks per minute but did not alter the temporal distribution of adjunctive behavior. Selective doses of dopaminergic antagonists further suppressed licking rates when combined with d-amphetamine, 1.0 mg/kg, but an antagonism could be observed on the temporal distribution after the administration of SCH23390 (0.01 mg/kg) or flupenthixol (0.1 mg/kg). The dopaminergic agonists SKF38393 ([R(+)-1-phenyl-7,8-dihydroxy-2,3,4,5-tetrahydro-1H-3-bezazepine hydrochloride]; 1.0-10.0 mg/kg) and quinpirole (0.003-0.03 mg/kg) produced a dose-dependent decrease in the rate of licking, and when combined with d-amphetamine, additive-like suppressive effects were found. These results suggest a dopaminergic selectivity for the effects of d-amphetamine on the temporal distribution of schedule-induced polydipsia.

Medication Adherence and Symptom Reduction in Adults Treated with Mixed Amphetamine Salts in a Randomized Crossover Study (Postgraduate Medicine) Objectives:

The study objectives were to 1) evaluate medication adherence for adults with attention-deficit/hyperactivity disorder (ADHD) treated with 3 times daily (TID) mixed amphetamine salts immediate release (MAS IR) versus once-daily (qAM) MAS extended release (MAS XR) in a randomized, crossover study; and 2) to examine the associations between adherence and efficacy for MAS IR and MAS XR.

Methods:

Sixty-two adults with ADHD were enrolled and 49 completed the study. The treatment condition order (TID-qAM or qAM-TID) was counterbalanced across participants, with an intervening washout period of ≥ 7 days. Adherence was assessed via 3 measures: 1) self-report, 2) pill count, and 3) the Medication Event Monitoring System (MEMS®). The primary efficacy measure was the ADHD Rating Scale (ADHD-RS); secondary measures included the Time-Sensitive ADHD Symptom Scale (TASS) and Clinical Global Impressions–Severity of Illness (CGI-S) scale.

Results:

Adherence to treatment as measured by self-report and pill count was not significantly different between MAS XR and MAS IR. Adherence was significantly better for MAS XR than MAS IR for all of the MEMS® measures. The mean change in ADHD-RS, TASS, and CGI-S scores at endpoint was significantly improved for both MAS IR and MAS XR and did not differ significantly between groups. There was not a significant adherence by efficacy interaction.

Conclusion:

Adults with ADHD adhered equally well with MAS IR as with MAS XR when assessed by pill count and self-report, but not by the MEMS® measures. Both treatments significantly reduced ADHD symptoms, and efficacy was not significantly different between groups. Adherence was not associated with treatment outcome.
