Mechanisms and standards exist to safeguard the health and welfare of the patient, but for electroconvulsive therapy (ECT) used to treat depression and other mental illnesses approval methods have failed. Prescribed to thousands over the years, public relations as opposed to medical trials have paved the way for this popular yet dangerous and controversial treatment option. Doctors of Deception is a revealing history of ECT (or shock therapy) in the United States, told here for the first time. J Neuropsychiatry Clin Neurosci. 2011 Sep 1; 23(4): E40 Curr Biol. 2012 Jan 18; BACKGROUND: When we view a scene, we construct an internal representation of the scene that extends beyond its given borders. This cognitive phenomenon is revealed by a subsequent memory error when we confidently misremember the extended scene instead of the original. This effect is known as "boundary extension" and is apparent in adults, children, and babies. RESULTS: Here we show that seven patients with selective bilateral hippocampal damage and amnesia, who cannot imagine spatially coherent scenes, displayed attenuated levels of boundary extension on three separate measures. Paradoxically, this reduced boundary extension resulted in better memory for the stimuli compared with matched control participants, because the patients' recall was less encumbered by the boundary extension error. A further test revealed that although patients could generate appropriate semantic, conceptual, and contextual information about what might be beyond the view in a scene, their representation of the specifically spatial aspect of extended scenes was markedly impoverished. CONCLUSIONS: The patients' superior memory performance betrayed a fundamental deficit in scene processing. Our findings indicate that the hippocampus supports the internal representation of scenes and extended scenes when they are not physically in view, and this may involve providing a spatial framework in scenes. We suggest that interference with the ability to internally represent space may prevent the construction of spatially coherent scenes, with possible consequences for navigation, recollection of the past, and imagination of the future, which depend on this function. Eur Neurol. 2012 Jan 18; 67(3): 155-160 Focal hippocampal diffusion-weighted imaging (DWI) lesion patterns are detected in transient global amnesia (TGA) patients in different frequency. It has been speculated that acute diffusion restrictions are associated with a worse outcome. Therefore, we evaluated the influence of acute DWI lesions on the cognitive long-term outcome in TGA patients. Seventeen otherwise healthy patients with the clinical syndrome of TGA, who had MRI investigations on admission as well as 1 day later, were investigated with a comprehensive neuropsychological test battery 2 years later. Acute hippocampal DWI lesions in TGA patients were detected in almost two thirds of the patients. Psychometric evaluation revealed no differences in cognitive performance between patients with and without DWI lesions as well as compared to healthy subjects. In addition, no relapse of the attack has been recognized in either group of TGA patients. Neuropsychol Rehabil. 2012 Jan 16; Cognitive rehabilitation research is increasingly exploring errorless learning interventions, which prioritise the avoidance of errors during treatment. The errorless learning approach was originally developed for patients with severe anterograde amnesia, who were deemed to be at particular risk for error learning. Errorless learning has since been investigated in other memory-impaired populations (e.g., Alzheimer's disease) and acquired aphasia. In typical errorless training, target information is presented to the participant for study or immediate reproduction, a method that prevents participants from attempting to retrieve target information from long-term memory (i.e., retrieval practice). However, assuring error elimination by preventing difficult (and error-permitting) retrieval practice is a potential major drawback of the errorless approach. This review begins with discussion of research in the psychology of learning and memory that demonstrates the importance of difficult (and potentially errorful) retrieval practice for robust learning and prolonged performance gains. We then review treatment research comparing errorless and errorful methods in amnesia and aphasia, where only the latter provides (difficult) retrieval practice opportunities. In each clinical domain we find the advantage of the errorless approach is limited and may be offset by the therapeutic potential of retrieval practice. Gaps in current knowledge are identified that preclude strong conclusions regarding a preference for errorless treatments over methods that prioritise difficult retrieval practice. We offer recommendations for future research aimed at a strong test of errorless learning treatments, which involves direct comparison with methods where retrieval practice effects are maximised for long-term gains. Neurocase. 2012 Jan 9; This paper reports the case of a patient, M.P., who developed delusion of inanimate doubles, without Capgras syndrome, after traumatic brain injury. His delusional symptoms were studied longitudinally and the cognitive impairments associated with delusion were investigated. Data suggest that M.P. did 'perceive' the actual differences between doubles and originals rather than 'confabulate' them. The cognitive profile, characterized by retrograde episodic amnesia, but neither object processing impairment nor confabulations, supports this hypothesis. The study examines the nature of object misidentification based on Ellis' and Staton's account and proposes a new account based on concurrent unbiased retrieval of semantic memory traces and biased recollection of episodic memory traces. Acad Emerg Med. 2012 Jan; 19(1): 2-10 ACADEMIC EMERGENCY MEDICINE 2012; 19:2-10 © 2012 by the Society for Academic Emergency Medicine ABSTRACT: Objectives: This study compared the clinical performance of the Canadian CT Head Rule (CCHR) and the New Orleans Criteria (NOC) for detecting any traumatic intracranial lesion on computed tomography (CT) in patients with a Glasgow Coma Scale (GCS) score of 15. Also assessed were ability to detect patients with "clinically important" brain injury and patients requiring neurosurgical intervention. Additionally, the performance of the CCHR was assessed in a larger cohort of those presenting with GCS of 13 to 15. Methods: This prospective cohort study was conducted in a U.S. Level I trauma center and enrolled a consecutive sample of mildly head-injured adults who presented to the emergency department (ED) with witnessed loss of consciousness, disorientation or amnesia, and GCS 13 to 15. The rules were compared in the group of patients with GCS 15. The primary outcome was prediction of "any traumatic intracranial injury" on CT. Secondary outcomes included "clinically important brain injury" on CT and need for neurosurgical intervention. Results: Among the 431 enrolled patients, 314 patients (73%) had a GCS of 15, and 22 of the 314 (7%) had evidence of a traumatic intracranial lesion on CT. There were 11 of 314 (3.5%) who had "clinically important" brain injury, and 3 of 314 (1.0%) required neurosurgical intervention. The NOC and CCHR both had 100% sensitivity (95% confidence interval [CI] = 82% to 100%), but the CCHR was more specific for detecting any traumatic intracranial lesion on CT, with a specificity of 36.3% (95% CI = 31% to 42%) versus 10.2% (95% CI = 7% to 14%) for NOC. For "clinically important" brain lesions, the CCHR and the NOC had similar sensitivity (both 100%; 95% CI = 68% to 100%), but the specificity was 35% (95% CI = 30% to 41%) for CCHR and 9.9% (95% CI = 7% to 14%) for NOC. When the rules were compared for predicting need for neurosurgical intervention, the sensitivity was equivalent at 100% (95% CI = 31% to 100%) but the CCHR had a higher specificity at 80.7% (95% CI = 76% to 85%) versus 9.6% (95% CI = 7% to 14%) for NOC. Among all 431 patients with a GCS score 13 to 15, the CCHR had sensitivities of 100% (95% CI = 84% to 100%) for 27 patients with clinically important brain injury and 100% (95% CI = 46% to 100%) for five patients requiring neurosurgical intervention. Conclusions: In a U.S. sample of mildly head-injured patients, the CCHR and the NOC had equivalently high sensitivities for detecting any traumatic intracranial lesion on CT, clinically important brain injury, and neurosurgical intervention, but the CCHR was more specific. Br J Anaesth. 2012 Jan 17; BACKGROUND: /st>Anaesthetics suppress the formation of lasting memories at concentrations that do not suppress perception, but it is unclear which elements of the complex cascade leading from a conscious experience to a lasting memory trace are disrupted. Experiments in conscious humans suggest that subhypnotic concentrations of anaesthetics impair consolidation or maintenance rather than acquisition of a representation (long-term more than short-term memory). We sought to test whether these agents similarly impair learning in rats. METHODS: /st>We used operant conditioning in rats to examine the effect of isoflurane on acquisition compared with long-term (24 h) memory of non-aversive olfactory memories using two different odour discrimination tasks. Rats learned the 'valences' of odour pairs presented either separately (task A) or simultaneously (task B), under control conditions and under isoflurane inhalation. In a separate set of experiments, we tested the ability of the animals to recall a learning set that had been acquired 24 h previously. RESULTS: /st>Under 0.4% isoflurane inhalation, the average number of trials required to reach criterion performance (18 correct responses in 20 successive trials) increased from 21.9 to 43.5 (P Behav Brain Res. 2012 Jan 18; The present work extends previous studies with the aim of developing the 4-vessel occlusion/internal carotid artery (4-VO/ICA) model of chronic cerebral hypoperfusion. The permanent occlusion of the vertebral arteries (VAs) and internal carotid arteries (ICAs) followed the sequence VA→ICA→ICA. The interstage interval (ISI, →), chronicity of 4-VO/ICA, and age of the animals may determine the success of the model with regard to neurohistological and behavioral outcomes. Using middle-aged rats, the present study evaluated (i) how brain damage evolves as the ISI is reduced and duration (i.e., "chronicity") of 4-VO/ICA is prolonged and (ii) how the duration of 4-VO/ICA affects retrograde memory performance. Male Wistar rats (12-15 months of age) were subjected to 4-VO/ICA with an ISI of 7, 5, 4, or 3 days, and hippocampal and cortical damage was examined 7, 30, and 90 days later. Using an ISI of 4 days, retrograde memory performance was assessed in the aversive radial maze after 4-VO/ICA with a duration of 7, 30, and 90 days. The severity of brain neurodegeneration and rate of mortality progressively increased as the ISI length decreased from 7 to 3 days, an effect that was not significantly altered by the chronicity of 4-VO/ICA. Permanent 4-VO/ICA effectively caused retrograde amnesia, an effect that worsened as the chronicity of 4-VO/ICA was prolonged. The findings confirm and expand the notion that permanent, 3-stage 4-VO/ICA effectively produces extensive neurodegeneration and persistent learning/memory impairment in middle-aged rats and that the ISI length, more than the chronicity of 4-VO/ICA, determines the final results. Neurol Sci. 2012 Jan 10; Anti-N-methyl-D: -aspartate receptor (NMDAR) encephalitis has been recently identified as a fulminant encephalopathy, presenting with a variety of symptoms including behavioral change, amnesia and seizures suggesting cortical gray matter involvement. A 42-year-old woman presented with acute-onset clinical and magnetic resonance imaging (MRI) findings indicating brainstem and diencephalon involvement. Her neuropsychological examination revealed mild frontal dysfunction with no memory impairment. Detailed diagnostic workup proved negative except for serum/cerebrospinal fluid (CSF) NMDAR-antibodies and increased activity in inguinal and pelvic lymph nodes on positron-emission tomography (PET) examination. The symptoms and MRI findings completely resolved following steroid treatment. A 38-year-old woman presented with migraine-type headache and episodes of forgetfulness. Her brain MRI and neuropsychological examination were normal and diagnostic workup was unremarkable. N-methyl-D: -aspartate receptor antibodies were identified in her sera and her symptoms spontaneously resolved within few months. Our cases suggest that anti-NMDAR encephalitis might present with minimal cognitive impairment, no apparent cortical gray matter involvement, a mild clinical course and without the classical clinical features of the disease. Neuroscience. 2012 Jan 18; Arising from studies on the amnesia that follows site-specific physical or chemical lesions, the acquisition and consolidation of certain behavioral tasks has been demonstrated to be associated with different hippocampal subregions. Although not absolute, spatial learning is reliant on the dorsal region of the hippocampus, whereas avoidance- and fear-conditioning tasks appear to be dependent on its more ventral aspects. Thus, if learning-associated synapse remodeling is a true feature of memory consolidation it must also follow these regional dissociations. We therefore determined if the learning-associated increases in synapse density that occur in the mid-molecular layer of the dentate gyrus at the 6-h post-training time and the frequency of polysialylated cells at the infragranular zone that occur at the 12-h post-training time were dissociated to specific hippocampal subregions following training in either a massed water maze task or light-dark passive avoidance response. Synapse remodeling was found to occur only in the dorsal hippocampus following spatial learning. We could not, however, discern any regional dissociation of neural remodeling following avoidance conditioning. These results point to strong associations between learning and specific groups of novel synapses during consolidation of spatial and avoidance conditioning paradigms. Turk Neurosurg. 2012; 22(1): 95-8 AIM: Plasma cell tumours include solitary plasmocytoma, multiple plasmocytoma and multiple myeloma. Herein we report a case of plasmocytoma of the skull. CASE HISTORY: A 49 year-old-man suffered from amnesia, irritability and a rubbery swelling which was gradually increased approximately to 4-5 cm in diameter at the frontal vertex over a year. His neurological examination was normal. The skull direct X-ray revealed patchy destruction of the left frontal bone 10 cm in diameter. CT with bone windows showed relatively preserved but severely thinned inner table and irregularly destructed outer table. MR scan showed a hypervascular, mainly solid mass measuring 9x5.5x8.5 cms. Radical surgery consisting of total tumour extirpation was undertaken. Histopathological examination confirmed a plasmocytic proliferation. The postoperative period was uneventful and the patient was discharged on the 6th day postoperatively. Bone marrow sampling was scheduled to rule out multiple myeloma, but the patient committed suicide a week later for reasons not known to us. CONCLUSION: Although the prognosis of a plasmocytoma is relatively good, this study demonstrated that patients with ventral frontal cortex tumour may have significantly worse mood pre- or postoperatively. Clinicians should therefore be careful in the follow-up period. Eksp Klin Farmakol. 2011; 74(10): 17-22 The neuroprotective activity of recombinant human erythropoietin (r-HuEpo) sorbed on poly(butyl)cyanoacrilate nanoparticles (EPO-PBCA) and on polylactic-co-glycolic acid nanoparticles (EPO-PLGA) has been studied on Wistar rats with intracerebral post-traumatic hematoma (model of hemorrhagic stroke) (IPH-HS) in comparison to native r-HuEpo. It is established that EPO-PBCA produced a protective effect in rats after IPH-HS that was manifested by a decrease in the number of animals with neurological disorders such as circus movement, paresis, and paralysis of hind limbs; the drug also improved coordination (rotating rod test), reduced the number of lost animals, and decreased the loss weight among survived rats. In addition, EPO-PBCA optimized the research behavior of rats with IPH-HS in the open field test and prevented amnesia of passive avoidance reflex (PAR), which was caused by the IPH-HS. These effects were manifested during a two-week observation period. EPO-PLGA has a similar but much less pronounced effect on the major disorders caused by IPH-HS. The efficiency of native r-HuEpo as a neuropotective agent was insignificant and only manifested by decrease in the number of lost animals with IPH-HS. Phytomedicine. 2012 Feb 1; This study assesses the efficacy of a fixed dose of Ropren(®) (a plant preparation isolated from the neutral fraction of an extract of spruce needles) on cognitive impairment in rats with β-amyloid peptide-(25-35)-induced amnesia. Ropren(®) was administered at a dose of 8.6mg/kg for 28 days, per os, to rats with β-amyloid peptide-(25-35)-induced amnesia. Cognitive performance was assessed using the passive avoidance paradigm and the Morris water maze and behavior was assessed using the open field test. After four weeks, Ropren(®) treatment significantly improved non-spatial and spatial learning in rats with β-amyloid peptide-(25-35)-induced amnesia. The results of the present study suggest that Ropren(®), a novel plant preparation, ameliorates cognitive deficiencies in an animal model relevant to Alzheimer's disease. Arch Med Sci. 2011 Feb; 7(1): 168-72 A 54-year-old diabetic, hypertensive man with poorly controlled moderate-severe sleep apnea presented with acute onset of severe anterograde amnesia and well preserved remote memory without additional cognitive impairment. Investigations, including a lumbar puncture, electroencephalogram (EEG) and serology testing ruled out infectious, neoplastic and epilleptogenic causes. MRI taken 10 days after symptom onset, was suggestive of sequential ischemic damage to both hippocampal formations. Neuropyschological evaluation suggested a focal and dense amnestic syndrome with little improvement over time. The bilateral nature of hippocampal ischemia though has been reported, is rare. It remains to be established how to examine these cognitive dysfunctions in the systematic way. "Action" deficit in dementia can occur at any stage from perception of objects to motor control. We examined the mechanism of "action" deficit in a patient with posterior cortical atrophy and that with progressive supranuclear palsy. The former demonstrated impaired control of visual attention, which seems to be related to dysfunction of the dorsal visual pathway. The latter showed poor control of ballistic action with a tool. Dysfunction of the supplementary motor area might be related to poor control of context-dependent ballistic action in this patient. Combined use of neuropsychological and neuroradiological data could clarify the underlying functional impairment and their neuronal bases, which will help understand pathological progression of atypical dementia. In addition to stand-alone measures, built-in, or embedded measures of effort have been derived for a limited number of standard neurocognitive tests. The Repeatable Battery for the Assessment of Neuropsychological Status (RBANS) is a widely used brief battery, employed as a core diagnostic tool in dementia and as a neurocognitive screening battery or tracking/outcome measure in a variety of other disorders. An effort index (EI) for the RBANS has been published previously (Silverberg, N. D., Wertheimer, J. C., & Fichtenberg, N. L. 2007. An EI for the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS). Clinical Neuropsychology, 21 (5), 841-854), but it has been reported to result in high false positive rates when applied to patients with "true" amnesia (e.g., Alzheimer's disease). We created a new effort scale (ES) for the RBANS based on the observation of patterns of free recall and recognition performance in amnesia versus inadequate effort. The RBANS ES was validated on a sample of patients with amnestic disorders and a sample of mild traumatic brain injury participants who failed a separate measure of effort. The sensitivity and specificity of the new ES was compared with the previously published EI. Receiver-operating characteristic analyses demonstrated much better sensitivity and specificity of the ES, with a marked reduction in false positive errors. Application and limitations of the RBANS ES, including indications for its use, are discussed. J Alzheimers Dis. 2012 Jan 18; Hormesis is the concept that low doses of a toxin can have beneficial effects while high doses are harmful. This is also known as the inverted-U shaped dose-response curve. Hormesis appears to be a universal law for the function of memory mimetics. Amyloid-β protein is widely recognized to be a toxic agent responsible for plaque formation in Alzheimer's disease. In high doses it also produces amnesia. In lower, physiological doses, it enhances long term potentiation and memory. Blocking amyloid-β protein in animals without overproduction of the protein results in amnesia. At low doses, amyloid-β also increases neurite outgrowth, produces presynaptic enhancement, and may quench oxidative damage. It is postulated that both over- and underproduction of amyloid-β can lead to memory deficits. This is similar to a number of hormonal diseases, e.g., thyroid, where both low and high levels produce disease. Scopolamine a muscarinic cholinergic antagonist is known to induce impairment in Morris water maze performance, the task which is mainly dependent on the hippocampus. It is suggested that hippocampal ERK and Akt activation play roles in synaptic plasticity and some types of learning and memory. Agmatine, a polyamine derived from l-arginine decarboxylation, is recently shown to exert some neuroprotective effects. This study was aimed to investigate if agmatine could reverse scopolamine-induced memory impairment and possible hippocampal ERK and Akt activity alteration. Adult male Sprague-Dawley rats weighing 200-250 g were randomly assigned into 5 groups. The animals were trained for 3 days in Morris water maze and in day 4 their memory retention was assessed in probe trial which was consisted of a 60 s trial with no platform. Scopolamine (1 mg/kg/ip) or saline were injected 30 min and agmatine (20 or 40 mg/kg/ip) was administered 60 min before each session. The hippocampi were isolated after behavioral studies and western blotting studies on hippocampal lysates were done to determine the levels of activated ERK and Akt. Scopolamine treatment not only impaired water maze learning and memory, but also decreased the amount of phosphorylated (activated) ERK and Akt. Agmatine pre-treatment prevented both the learning impairment and hippocampal ERK and Akt inactivation induced by scopolamine. It seems that agmatine may act as a candidate substance against amnesia. Memory. 2012 Feb; 20(2): 189-97 This within-culture study aimed to investigate the age and density of earliest memories in a sample of Italian young adults. The framework of this study is sociocultural, with an emphasis on family reminiscing as contributing to early memories. In this regard Italy is of interest due to the fact that it is a Western but familial oriented culture, where multi-generational family ties are still strong in comparison with other European counterparts, providing for the opportunity to investigate the offset of childhood amnesia as a function of growing up in an extended family structure. Consistent with Mullen (1994), 90 undergraduate students were interviewed about their earliest memory and about sociodemographic factors in their early childhood years: household composition, gender, birth order, presence of family moves, and significant early childhood experiences. Participants who grew up in an extended family situation reported earlier and denser memories in comparison with those from nuclear families. Results are discussed in light of other cross-cultural work on childhood amnesia and with respect to sociocultural theories of the importance of family reminiscing for one's early memories. Brain. 2012 Jan; 135(Pt 1): 242-58 Contemporary clinical and basic neuroscience studies have increasingly implicated the anterior temporal lobe regions, bilaterally, in the formation of coherent concepts. Mounting convergent evidence for the importance of the anterior temporal lobe in semantic memory is found in patients with bilateral anterior temporal lobe damage (e.g. semantic dementia), functional neuroimaging and repetitive transcranial magnetic stimulation studies. If this proposal is correct, then one might expect patients with anterior temporal lobe resection for long-standing temporal lobe epilepsy to be semantically impaired. Such patients, however, do not present clinically with striking comprehension deficits but with amnesia and variable anomia, leading some to conclude that semantic memory is intact in resection for temporal lobe epilepsy and thus casting doubt over the conclusions drawn from semantic dementia and linked basic neuroscience studies. Whilst there is a considerable neuropsychological literature on temporal lobe epilepsy, few studies have probed semantic memory directly, with mixed results, and none have undertaken the same type of systematic investigation of semantic processing that has been conducted with other patient groups. In this study, therefore, we investigated the semantic performance of 20 patients with resection for chronic temporal lobe epilepsy with a full battery of semantic assessments, including more sensitive measures of semantic processing. The results provide a bridge between the current clinical observations about resection for temporal lobe epilepsy and the expectations from semantic dementia and other neuroscience findings. Specifically, we found that on simple semantic tasks, the patients' accuracy fell in the normal range, with the exception that some patients with left resection for temporal lobe epilepsy had measurable anomia. Once the semantic assessments were made more challenging, by probing specific-level concepts, lower frequency/more abstract items or measuring reaction times on semantic tasks versus those on difficulty-matched non-semantic assessments, evidence of a semantic impairment was found in all individuals. We conclude by describing a unified, computationally inspired framework for capturing the variable degrees of semantic impairment found across different patient groups (semantic dementia, temporal lobe epilepsy, glioma and stroke) as well as semantic processing in neurologically intact participants. |
