This guide is one of two that the author is aware of that guides protégés through the steps in four sessions, but the other guide uses the Oxford Group's more religious approach where this guide offers techniques that were used by some of the early founders and sponsors and focuses more on AA principles alone.

The Big Book reports that the founders guided protégés through the steps in "three or four hours." Some feel that the Big Book could not possibly be stating the truth about that time line, but we found from interviews with some of the early members that such was the case.

You may first want to use the guide yourself; then, in your twelfth-step work with others, you can use this guide one-on-one as you go through The Work. Eventually, after using the guide several times, you will likely find that you have mastered the process and can take people through all twelve steps without the guide at all.

The guide is divided into four parts which take an hour or so each, and while we had doubts, Mark and the author tried it and it worked! As Mark said, “This process need not be long and arduous.”

Background: Although oxycodone is one of the most widely available and abused opioids, little published information exists on the abuse of immediate-release oxycodone. Objective: To obtain information on abuse of oxycodone and the effectiveness of abuse-deterrent strategies, especially for immediate-release oxycodone, we surveyed oxycodone abuse patterns in a population of experienced opioid abusers. Methods: Students or recent graduates of two substance abuse recovery high schools in Massachusetts were surveyed on abuse behaviors with short-acting single-entity oxycodone (e.g., Roxicodone), short-acting combination oxycodone (e.g., Percocet), and extended-release oxycodone. Results: Twenty-four students completed surveys. Mean age was 17.7 years (range 16-19), and mean age at first abuse of oxycodone was 15 (range 13-18). Overall, 56% of students reported oxycodone as their favorite prescription opioid to abuse. The primary preferred method of abuse of all oxycodone formulations was intranasal administration: 83% of single-entity oxycodone abusers preferred intranasal administration compared with 67% of combination oxycodone abusers and 69% of extended-release oxycodone abusers. Approximately half of our respondents preferred to ingest oxycodone orally, 25-38% of respondents swallowed the pill intact, and another 13-17% chewed the pill before swallowing. Maximum dose ever abused at one time ranged from 15 to 400 mg. Most respondents had abused ≥60 mg of oxycodone at a time. Conclusions: In this small study, adolescent oxycodone abusers use high quantities of oxycodone at a time, alter routes of administration for not only extended-release but also immediate-release products, and commonly abuse single-entity oxycodone products. Abuse-deterrent formulations may be one strategy for addressing such behaviors.

This discussion piece suggests that reliance on a Higher Power in drug abuse recovery programs is entertained among some addicts for its psychobiological effects. Prayer, meditation, early romantic love, and drug abuse may have in common activation of mesolimbic dopaminergic pathways of the brain and the generation of intense emotional states. In this sense, reliance on a Higher Power may operate as a substitute addiction, which replaces the psychobiological functions formerly served by drug use. Implications of this perspective are discussed.

This study compared the social climate of peer-run homes for recovering substance abusers called Oxford House (OH) to that of a staffed residential therapeutic community (TC). Residents of OHs (N = 70) and the TC (N = 62) completed the Community Oriented Programs Environment Scales. OHs structurally differ on two primary dimensions from TCs in that they tend to be smaller and are self-run rather than professionally run. Findings indicated significantly higher Involvement, Support, Practical Orientation, Spontaneity, Autonomy, Order and Organization, and Program Clarity scores among the OH compared to TC residents. Additional analyses found the OH condition was higher Support, Personal Problem Orientation, and Order and Organization scores among women compared to men residents. These results suggested that these smaller OH self-run environments created a more involving and supportive social milieu than a larger staff-run TC. These findings are interpreted within Moos' (2007) four theoretical ingredients (i.e., social control, social learning, behavioral economics, and stress and coping), which help account for effective substance abuse treatment environments.

There is a lack of consensus in the addiction treatment literature regarding the definition of substance abuse "recovery".This study utilized a review of the literature together with a participatory research design to construct a conceptual model of recovery from the perspectives of addiction treatment professionals, those recovering from addictions, and researchers.A multidimensional, comprehensive hypothetical model consisting of seven conceptual domains (physical, biomarker, psychological, psychiatric, chemical dependency, family/social, and spiritual) is presented. Each domain is operationally defined by identifying reliable and valid instruments that may be used to measure the domain. It is proposed that the conceptual model be tested using confirmatory factor analysis.If empirically supported, this conceptual model would validate the hypothesized multidimensional nature of recovery and provide a potential means for assessing recovery in future treatment outcome studies.

Women currently are 30% of the substance abuse recovery population in North America and have gender specific treatment needs as they enter the difficult work of recovery. Important among women's specific needs as they enter recovery is the need for a focus on primary health care. Few models designed to guide the provision of health care for this population are available in the literature. The Tidal Model of Mental Health Recovery and Reclamation is based on the concept of nursing as "caring with" persons in the experience of distress. Given the emphasis in this model on developing a partnership between caregiver and client, it is especially appropriate for women in recovery for substance abuse. The Tidal Model, integrated with the United States Substance Abuse and Mental Health Services' CSAT model for comprehensive alcohol and other drug (AOD) abuse treatment, is used to guide planning for delivery of primary health care in a residential women's substance abuse recovery center in the Midwest. This article describes the Tidal Model, and identifies how the model can improve the delivery of primary care to women in residential substance abuse treatment. Strategies for implementation of the model are proposed. Evaluation and outcome criteria are identified.

Bioethics and public policy literature suggests that socially marginalized populations may be at increased risk for overuse of physician-assisted suicide (PAS) were it to become more accepted. Yet the attitudes of socially marginalized populations toward PAS have not been widely studied. The present study surveyed a sample of men in a substance abuse recovery program. Participants completed a PAS attitude questionnaire and a religiosity measure. Support for PAS was fairly evenly split, with 52.2% indicating general opposition to PAS and 46.9% indicating general support. Greater religiosity was associated with more negative attitudes toward PAS. Higher educational level was associated with more acceptance. Overall attitudes toward PAS were considerably more negative than those of Dr. George Domino's (2003) general population sample.

HIV/AIDS is recognized as affecting and being affected by the family. HIV+ women in drug recovery and their families are particularly at risk due to family disruption and stigma. Yet family research with HIV+ adults is hampered by the challenges of defining the family, engaging family members into research, and tracking changes in family composition. In this paper we describe the family context of 144 HIV+ women in drug abuse recovery who are enrolled in a randomized trial of a family intervention to improve medication adherence and reduce relapse. Family was defined to include the women's household members, romantic partners, children and their caregivers, and others identified as a major source of support. The women reported on a total of 651 family members. We describe the family and household networks, romantic partnerships, and parenting arrangements of our participants. We also describe family member enrollment in the research study and the stability of romantic partnerships, parenting, and living arrangements over 1 year. We conclude with methodological implications for future family based clinical research with HIV+ adults.

To address the lack of research examining within-person processes associated with the maintenance of abstinence from substance use, this study examines both associations between negative affect and social experiences and same-day levels of substance use cravings among college students in 'Twelve-Step' substance abuse recovery and the role of coping strategies in moderating these within-day associations. The sample consisted of 1222 end-of-day reports made by 55 recovering college students, 39 males and 16 females. Findings include that daily cravings were predicted by same-day negative affect and each of four negative social experiences. Moreover, each of these within-day associations was moderated by individuals' levels of avoidance coping. In contrast, problem-solving coping only moderated the associations between negative affect and cravings. These findings document a within-day mechanism through which coping strategies, especially avoidance coping, may influence daily variation in cravings and in turn affect abstinence.

Researchers have identified four common co-occurring parental risk factors-substance abuse, mental illness, domestic violence, and child conduct problems-that lead to child maltreatment. The extent to which maltreatment prevention programs must directly address these risk factors to improve responsiveness to parenting programs or can directly focus on improving parenting skills, says Richard Barth, remains uncertain. Barth begins by describing how each of the four parental issues is related to child maltreatment. He then examines a variety of parent education interventions aimed at preventing child abuse. He cautions that many of the interventions have not been carefully evaluated and those that have been have shown little effect on child maltreatment or its risk factors. Although some argue that parent education cannot succeed unless family problems are also addressed, much evidence suggests that first helping parents to be more effective with their children can address mental health needs and improve the chances of substance abuse recovery. Barth recommends increased public support for research trials to compare the effectiveness of programs focused on parenting education and those aiming to reduce related risk factors. Child welfare services and evidence-based parent training, says Barth, are in a period of transformation. Evidence-based methods are rapidly emerging from a development phase that has primarily involved local and highly controlled studies into more national implementation and greater engagement with the child welfare system. The next step is effectiveness trials. Citing the importance and success of multifaceted campaigns in public health policy, Barth discusses a multifaceted parenting campaign that has demonstrated substantial promise in several large trials. The goal of the Triple P-Positive Parenting Program is to help parents deal with the full gamut of children's health and behavioral issues. The campaign includes five levels of intervention, each featuring a different means of delivery and intensity of service. More broadly, Barth suggests that the evidence-based Triple P approach offers a general framework that could be used to guide the future evolution of parenting programs.

Mothers who are dually-diagnosed with HIV/ AIDS and drug abuse are particularly burdened and vulnerable to loss of child custody.This study explores factors associated with child custody loss among HIV+ women in drug abuse recovery, and compares mothers who have custody of their children to mothers who do not have custody of all of their children.Descriptive analyses were conducted on 68 HIV+ mothers in drug recovery, 46% who had custody of all of their children, and 54% who did not. Results: The lost-custody group was less likely to be employed and had more history of violent impulses and arrest, particularly on drug charges. The groups reported similar recent and lifetime drug use patterns, but the custody group had received more drug treatment. Post-hoc analyses found the lost-custody group had lower rates of participation in the interventions offered in the parent clinical trial and higher avoidant coping.The sample in the current study supports that HIV+ women in drug recovery are at high risk of loss of child custody. Non-adherence to available treatment seems to be associated with loss of child custody.Results suggest the need for further research and active treatment outreach for this population.

The National Institute of Drug Abuse has promoted drug abuse research in the past two decades focusing on women and gender differences. One hundred twenty-eight Hispanic and White women have participated in this comparative descriptive study that has examined the differences between chemically dependent (CD) women in recovery and non-chemically dependent (non-CD) women in regard to resilience and self-differentiation-demographic variables associated with resilience and self-differentiation and recovery variables associated with resilience and self-differentiation in the CD women. Findings indicate that the CD women and Hispanic women have scored significantly lower on measures of resilience and self-differentiation. Among the recovery variables, resilience and self-differentiation are significant for children support but community support is not significant. The finding that Hispanic and White women in recovery score lower on resilience and self-differentiation is important for designing treatment strategies supportive of women in recovery.

This study identified and characterized groups of clients who have serious mental illness and co-occurring substance use disorders.Substance abuse recovery of 177 clients with co-occurring disorders was followed for ten years with multiperspective ratings every six months. Latent-class trajectory analysis was used to identify subgroups; profile analysis was used to examine baseline risk factors and treatment participation.Two ratings of substance abuse recovery-stage of treatment and abstinence-were highly concordant and identified the same four groups: early recovery, unstable recovery, late recovery, and no recovery. The early-recovery group was characterized by less severe substance use disorders at baseline and by use of clozapine. The unstable-recovery group was too small for statistical analyses. The late-recovery group was similar to the no-recovery group at baseline but participated more extensively in treatments, especially residential dual-diagnosis programs, during the first three years of follow-up.Latent-class trajectory analysis based on ten-year trajectories of substance abuse recovery identified clinically meaningful groups among clients with serious mental illness and co-occurring substance use disorders. Profiles of the three most common groups suggest differential clinical approaches for each.

Many health-care providers may overlook or be unaware of most drug-to-drug interactions. Recognizing the existence of drug interactions with cigarette smoking and alcohol ingestion can empower a clinician with knowledge to avoid dangerous interactions that may result in hazardous, negative patient outcomes. Cigarette smoking and alcohol use can reduce the efficiency of certain drugs or make drug therapy more unpredictable.This review offers the physician information regarding prescription drug interactions with cigarette smoking and alcohol use. First, mechanisms found in the medical literature of potential drug interactions in cigarette smokers and alcohol drinkers are presented. Second, the 100 most frequently prescribed medications in 2006 are reviewed regarding cigarette smoking effects and alcohol effects as cited in the medical literature. Lastly, a table of these 100 medications and any reported effects of cigarette smoking or alcohol consumption on each drug is provided.The actual number of different medications reviewed was 78. Drug interactions resulting from the effects of cigarette smoking occurred with 33.3% of the drugs (n = 26), and drug interactions resulting from the effects of alcohol consumption occurred with 76.9% of the drugs (n = 60). Finally, resource information regarding smoking cessation and alcohol abuse recovery is summarized so that physicians may empower their patients to avoid potential drug-interaction events.Cigarette smoke and alcohol may interact with medications through pharmacokinetic or pharmacodynamic mechanisms. Engaging in both of these social activities can reduce the efficiency of certain drugs or can make drug therapy unpredictable. This review offers the health-care provider information regarding potential prescription drug interactions. Empowered with this information, clinicians may assist their patients to maximize pharmacologic outcomes by avoiding these reported harmful interactions.

In addition to being a widely used and effective approach for alcohol problems, AA has been central to the development of several types of nonprofessional recovery programs. Known as "social model recovery," these programs were staffed by individuals in recovery and they encouraged program participants to become involved in AA as a way to address their drinking problems. In addition, they relied on the traditions, beliefs, and recovery practices of AA as a guide for managing and operating programs (e.g., democratic group processes, shared and rotated leadership, and experiential knowledge). This chapter reviews the philosophy, history, and recent changes in several types of these programs, along with a depiction of AA's influence on them. Programs examined include neighborhood recovery centers, residential social model recovery programs, and two types of sober living houses: California Sober Living Houses and Oxford Houses. Recent outcome evaluations on both types of sober living houses are presented.

The present study compared the characteristics of individuals living with (42 men, 52 women) and without children (561 men, 241 women) residing in an communal-iving recovery program called Oxford Houses.. Results indicated that men living with children and women living without children had more general social support, compared to men living without children and women living with children. Additionally, women and residents of adult-only houses reported having more drug users in their social networks. However, men and women living with and without children reported similar levels of social support for abstinence. It is suggested that that men in recovery who take care of their children are in situations more advantageous to sustained recovery and have more resources compared to recovering women with children. Women in substance abuse recovery and taking care of children may require additional resources and assistance compared to men.

Two examples of mutual-help approaches for substance abuse recovery are 12-step groups (AA and NA) and Oxford House. The present study examined the combined effects of AA and Oxford House residence on abstinence over a 24-month period with 150 individuals randomly assigned to either an Oxford House or to usual after-care. Among individuals with high 12-step involvement, the addition of Oxford House residence significantly increased the odds of abstinence (87.5% vs. 52.9%). However, among participants with low 12-step involvement, rates of abstinence were fairly similar across conditions (31.4% vs. 21.2%). Results suggested that the joint effectiveness of these mutual-help programs may promote abstinence.

With a national U.S. sample of communal-living residents in substance abuse recovery, the tendency to help members inside and/or outside their community was examined. Study 1 (n = 670) developed of the Communal Living In-Group Helping Scale to distinguish helping directed toward housemates vs. others. Study 2 (n = 419) used this communal helping measure and a general altruism scale to explore gender, ethnicity, and 12-Step sponsorship related to in-group (housemates) and out-group (others in the community) behaviors. Results revealed significant sex differences and significantly higher helping for both men and women was reported among 12-Step sponsors along two dimensions. Implications focused on gender-related differences in social helping interactions and in-group formation in recovery communities.

This study examined employment and sources of income for different genders and ethnic groups residing in a substance abuse recovery homes called Oxford Houses. Men compared to women reported significantly higher mean income from employment as well as total income. African Americans compared to European Americans reported significantly more work in the past 30 days; however, the rate of pay between these two ethnic groups was not significantly different. Longer length of stay in Oxford House was related to higher incomes. Implications of these findings are discussed.

Hope is commonly divided into two constructs: agency, defined as goal-directed energy, and pathways, defined as the ability to create paths to a goal. To date, only two studies have examined the utility of hope in substance abuse recovery, and the present investigation buildings on this small literature by assessing hope beliefs within a larger and more diverse sample of adults in recovery. This study examined how two hope constructs of agency and pathways related to substance use abstinence among 90 new residents of communal-living recovery homes (i.e., Oxford Houses) who completed two waves of data assessment. Results indicated that agency scores significantly predicted alcohol use at Wave 1 but pathway scores failed to predict drug or alcohol use at this time point. Additionally, agency and pathway scores predicted drug (but not alcohol use) at an 8-month follow-up assessment. These findings indicated that participants' hope may be linked to substance use at later stages of recovery. In addition, these results suggested a stronger relationship between hope and drug as opposed to alcohol use at this time point. Implications for substance abuse recovery are discussed.
